This Additional Service - SIRCA (SafetyWallet Incident Root Cause Analysis) is a step-by-step investigation to determine the root cause of an incident to ensure that procedures can be set in place to prevent that incident from happening again.
What to expect of SIRCA Incident Investigations
You can expect a thorough investigation of the incidents and a detailed description of the measures to be put into place to prevent the incident from recurring.
After the SIRCA has been completed, the Control Measures in the Task Risk Assessment must be updated by the Health and Safety Representative, implimented and monitored.
